Food insecurity rarely exists in isolation. Many families navigating hunger are also eligible for SNAP benefits or Medicaid coverage that would stabilize their household. One Heart’s benefits navigation program connects neighbors with the information, application support, and resource navigation that reduce barriers to essential public benefits — a core strategy in addressing the social determinants of health.

Benefits assistance appointments are available every Friday. Appointments accommodate up to 4 clients per hour. Choose a service below — SNAP/Food Stamp assistance or Medicaid navigation — and select an available Friday time slot.
Exact Friday service-window start and end times are staff-configurable and must be completed before this calendar is published.
